Ham Sandwich to Toe Hold

The Ham Sandwich (aka The Bear Trap) is not necessarily an unheard of play in Jiu Jitsu, but it is SUPER rare to see someone that actually has a system and entry and exit points built around it. The most obvious counter is for your opponent to drive their knee across and cut the angle. When that Happens, switch off to the Toe Hold and go for the leg lock.
